Welcome to Pondera and thanks for choosing us to help you with your health needs! Below is some information to help you prepare for your first appointment with us. We look forward to meeting and helping you!
Location & Parking
West End: We’re located at Level 1, 162 Boundary Street, West End. We provide free parking at the rear of the building in West Point car park – the entrance is accessible from Boundary Street and is located between the Night Owl building and our building. We are also easily accessible by public transport (closest bus stops are serviced by the 198 & 199 bus routes). While the limit on free parking is 2 hours, we can provide clinic passes for those who are here longer than two hours ie: pilates session followed by a massage.
North Lakes: We’re located at Suite 2B Lakes Vista Office Park, 2 Flinders Parade, North Lakes. There is free parking in the on-site underground car park with stair access to street level. Drive in the driveway, and proceed down the driveway for 50 metres, keep going to left and you will see the ramp down to the car park. There is also a disabled car space out the front of the building plus 15 minute parking on Flinders Parade.
Access to the Building
West End: The building front door and carpark are open from 7am to 6pm Monday – Friday, and until 12pm on Saturday. There is a buzzer entrance for the front door (dial 12 and then press the bell) before and after these hours, however the lift from the carpark is not accessible outside those times.
North Lakes: The building front door and the carpark entrance/exit is open until 6pm Monday to Friday and until midday on Saturdays. The building and our clinic is wheelchair accessible.
What to bring
Please review the below checklist and bring along all relevant items to your appointment:
- A Doctor’s referral or other referrals or reports related to your condition
- Recent X-rays, CT, ultrasound or MRI scans related to your condition
- List of current medications
- Private health fund card if you have private health extras cover
- Cash, debit card or credit card
For DVA members:
- DVA Card
For Workcover claims:
We can only invoice Workcover directly if your claim has been lodged and approved. A claim number alone is not enough for this to occur.
We charge a gap fee between the Workcover fees and our usual private fees (currently $20 for the initial consult and $18 for subsequent sessions).
- Your current Workcover medical certificate covering the period required for Physiotherapy treatment and specifically requesting Physiotherapy
- Case Manager name and contact details if known
For EPC / GP Management Plan patients:
Please note we charge a gap fee for Physiotherapy services between the Medicare rebate of $52.95 per treatment session and our usual private fees. This gap fee is waived for Pension and health care card holders. Please bring:
- Medicare card and debit card for Medicare rebate processing
- EPC / GP Management Plan paperwork: (if not already provided to us)
- GP and relevant medical specialist details: To complete our patient registration form, please have these details on-hand
What to wear
Please wear or bring along something you can comfortably move in ie: activewear, shorts & singlet.
A thorough clinical assessment will generally look at the whole body in motion, not just a specific painful region.
New Patient Registration form
This link is to our new patient registration form. If possible, please download the form, complete the relevant information and either bring along to your appointment, or email it back to us at email@example.com for the West End clinic or firstname.lastname@example.org for the North Lakes clinic. Otherwise, please allow an extra 10 minutes before your first appointment to fill in this form at the clinic.
Duration of your appointment
For your first appointment, we allow 30 minutes for short term conditions (generally under 4 weeks), DVA, EPC and Workcover claims.
We allow 45 minutes for all other conditions to undertake a thorough assessment and discuss a treatment plan with you.
For subsequent follow-up appointments, we allow 30 minutes.
Some complex conditions or treatment of multiple areas may require longer consultations and if that is the case, your Physiotherapist or Exercise Physiologist will discuss with you.
We know your time is valuable and we appreciate that you understand that ours is too. If you need to cancel and reschedule an appointment, please provide us with 24 hours notice to avoid a possible cancellation fee.
This fee will be 50% of the service fee for appointments, or a full session charge for Pilates studio sessions.
